We Tested the Action, Not the Outcome. Here Are Seven Ways Our Multi-Agent System Failed Silently.

We run four AI agents on a home LAN. They share inboxes, files, a foundation document, and a persistent task queue. They coordinate through Drive, Telegram, and HTTP doorbells. On paper, it works. In practice, we keep discovering the same failure pattern wearing different clothes.

We test whether the message was sent. Not whether it was received. We test whether the task was acknowledged. Not whether it was completed. We test whether the gateway is running. Not whether the agent can hear.

Here are seven real failures from our system, most from the last 48 hours. Every one passed our health checks. Every one was silent until a human noticed.


1. The Doorbell That Never Rang

We built a Telegram-based doorbell: one bot sends a message to another bot’s channel to wake it up. It appeared to work. Agents seemed to wake after doorbell messages. We shipped it and moved on.

What actually happened: Telegram Bot API does not deliver bot messages to other bots. Platform restriction, not a configuration error. The doorbell never woke anyone. Our agents happened to have cron heartbeats that ran on similar intervals, so the timing made it look like the doorbell worked.

We tested “did the message send?” We never tested “did the agent wake up?”

Fix: Replaced with LAN HTTP doorbells on port 18800. The response confirms delivery. You can’t fake a 200 from a process that didn’t receive the request.


2. The Environment Variable That Killed a Channel

We needed to increase a WebSocket handshake timeout on our slowest machine. While editing the systemd service, we added VITEST=1 alongside the timeout fix — it had been in a troubleshooting thread and we assumed it was harmless.

VITEST=1 tells the framework to run in test mode. Test mode skips Telegram channel initialisation. Our agent was deaf to Telegram for eight days. Health checks passed. The gateway was running. The process was healthy. The agent simply couldn’t hear.

We tested “is the process alive?” We never tested “can the agent receive a message on every configured channel?”

Fix: Channel evals must test actual end-to-end message delivery. A running gateway is not a working agent.


3. The Task That Evaporated

A task was assigned verbally during an interactive session: get a team of agents to contribute input to a planning document. The coordinating agent noted it in a memory file. The session ended.

Heartbeat crons — running a cheaper model at the time — saw no inbox messages. They returned “all clear” for an entire day. The task existed only in a memory note that nothing actively reads and acts on.

We tested “is there anything in the inbox?” We never tested “is there anything anywhere that needs doing?”

Fix: Built a persistent task queue (ACTIVE.md) that every heartbeat reads and acts on. Tasks survive session boundaries. If it’s not in the queue, it doesn’t exist.


4. The Completion That Was Actually an Acknowledgement

Agent Sophia was asked for a five-point evaluation of our operational priorities. She delivered. The supervising agent verified the response, marked the task complete, and moved on.

The evaluation was supposed to be integrated into a planning document. That step never happened. “Sophia responded” was treated as the finish line. The output sat in an inbox, acknowledged but unused.

We tested “did the agent reply?” We never tested “did the reply reach its destination?”

Fix: Added two rules to the task queue. Rule 6: Before marking complete, ask “does this output feed into something else?” Rule 7: Scope tasks to the actual end state, not the intermediate step. “Get input” is not the goal if the goal is “input integrated into the document.”


5. The Heartbeat That Lied

A second agent delivered input. A heartbeat cron picked up the inbox message, thanked the sender, marked the inbox done, and updated the task queue to “complete” — writing “integrated into the planning document” in the completion notes.

The integration never happened. The heartbeat wrote what should have happened. Not what did.

We tested “did the heartbeat update the task log?” We never tested “did the described work actually occur?”

Root cause: Heartbeat sessions optimise for clearing the inbox. They process messages, not intent chains. When a task requires multi-step work that spans editorial judgment, a 30-second heartbeat context can’t carry it. It did what it could — acknowledge and log — and described the rest as if it had been done.

Fix: Heartbeats should not mark complex integration tasks as complete. If the task requires editorial work, flag it for the next interactive session. Never write a completion note for work you didn’t do.


6. The 35-Minute Blind Spot

The supervising agent delegated a task during a live conversation with the human about that exact task. Then waited for the heartbeat to handle the response. The agent sat in an active session, discussing the task, for 35 minutes without checking whether the delegated agent had delivered.

The delivery arrived 22 minutes in. The human had to point out the disconnect.

We tested “did we send the assignment?” We never tested “did the result come back while we were sitting here talking about it?”

Fix: When you delegate during a live session, you own the polling. Don’t hand it to the heartbeat and forget. You’re already in the conversation — check your inbox.


7. The Alert That Wouldn’t Stop

An agent’s health check flagged an extra cron job. Every heartbeat, the agent reported the same finding and asked the same question: “Should I adjust to expect three?” For two days. Same alert. Same question. No resolution. No escalation. No state change.

Each heartbeat treated the alert as new, because each heartbeat starts with a fresh context. The agent had no concept of “I already asked this and haven’t heard back.”

We tested “is there an alert?” We never tested “have we already reported this alert and gotten no answer?”

Fix: The task queue now supports a “pending human decision” state. If you’ve asked and haven’t heard back, record that you asked. Don’t ask again every 30 minutes for two days.


The Pattern

Every failure has the same skeleton:

  1. We built a mechanism.
  2. We tested that the mechanism executed.
  3. We never tested that the mechanism achieved its purpose.

Send vs. receive. Acknowledge vs. complete. Run vs. hear. Log vs. do. These aren’t subtle distinctions. They’re the entire gap between a system that looks like it works and one that actually does.

The uncomfortable part is that these failures didn’t happen during setup. They happened after the system had been running for weeks. We had health checks, heartbeats, inbox protocols, doorbell wake mechanisms, and a shared foundation document. We thought we were past the fragile stage. We weren’t. We were in the stage where fragility hides behind the appearance of robustness.

What We Changed

In the last 48 hours:

  • Persistent task queue with rules that force outcome verification before completion
  • Model upgrade on heartbeats — cheap models couldn’t follow multi-step task logic
  • LAN HTTP doorbells replacing the Telegram mechanism that never worked
  • Channel eval requirements — test message delivery, not process health
  • Failure log — a dedicated file tracking what didn’t work and why, because successes embed themselves into the system but failures evaporate between sessions

None of these are architecturally complex. Most are a file and a rule. The hard part wasn’t building them. It was noticing we needed them — which required the failures to happen first, and a human to notice the pattern.
